RESUMO

Objective Structured Clinical Examination (OSCE) é um modelo de avaliação com uma série de estações, nas quais o aluno deverá executar ações específicas que englobam competências. São avaliados comportamento, domínio e habilidades. Objetivo: relatar a utilização da simulação realística como ferramenta de avaliação de residentes de enfer- magem. Método: Estudo descritivo, tipo relato de experiência, sobre o uso de um método de avaliação formativa utilizando a simulação realística em julho de 2023. Resultados: Foi realizada a observação direta das atividades desenvolvidas pelos residentes e preenchido o check list individual de cada cenário. Debriefing contribuiu com a discussão reflexiva em grupo. Conclusão: Ao acompanhar o desenvolvimento enquanto ele ocorre, a avaliação formativa com uso do método OSCE, possibilita perceber como o residente tem absorvido o saber, quais possíveis pontos de melhoria e se os objetivos estão sendo alcançados. A simulação utilizada como ferramenta pode complementar o treinamento prático em situa- ções clínicas reais.(AU)


Objective Structured Clinical Examination (OSCE) is an assessment model with a series of stations in which the student must perform specific actions that encompass competencies. Behavior, mastery and skills are assessed. Objective: To report on the use of realistic simulation as an assessment tool for nursing residents. Method: This is a descriptive experience report on the use of a formative assessment method using realistic simulation in July 2023. Results: The activities carried out by the residents were directly observed and an individual checklist was completed for each scenario. Debriefing contributed to reflective group discussion. Conclusion: By monitoring development as it happens, formative assessment using the OSCE method makes it possible to see how the resident has absorbed the knowledge, what possible points for improvement and whether the objectives are being achieved. Simulation used as a tool can complement practical training in real clinical situations.(AU)


El Examen Clínico Objetivo Estructurado (ECOE) es un modelo de evaluación con una serie de estaciones en las que el estudiante debe realizar acciones específicas que engloban competencias. Se evalúan el comportamiento, el dominio y las habilidades. Objetivo: Informar sobre el uso de la simulación realista como herramienta de evaluación para residentes de enfermería. Método: Estudio descriptivo del uso de un método de evaluación formativa mediante simulación realista en julio de 2023. Resultados: Se observaron directamente las actividades realizadas por los residentes y se cumplimentó una lista de comprobación individual para cada escenario. El debriefing contribuyó a la discusión reflexiva en grupo. Conclusión: La evaluación formativa mediante el método OSCE permite, a través del seguimiento de la evolución a medida que ésta se produce, ver cómo el residente ha asimilado los conocimientos, cuáles son los posibles puntos de mejora y si se están alcanzando los objetivos. La simulación utilizada como herramienta puede complementar la formación práctica en situaciones clínicas reales.(AU)

RESUMO

BACKGROUND: After the COVID-19 epidemic, the state has paid more attention to the clinical teaching function of affiliated hospitals of colleges and universities. Strengthening the integration of medicine and education and improving the quality and effect of clinical practice teaching are critical challenges facing medical education. The difficulty of orthopedic teaching lies in the characteristics of a wide variety of diseases, strong professionalism, and relatively abstract characteristics, which affect the initiative, enthusiasm, and learning effect of nursing students. In this study, a flipped classroom teaching plan based on the CDIO (conceive-design-implement-operate) concept was constructed and practiced in the orthopedic nursing student training course to improve the effect of practical teaching, and it is convenient for teachers to implement more effective and targeted teaching in the flipped classroom of nursing education and even medical education in the future. METHODS: Fifty undergraduate nursing students who practiced in the Orthopedics Department of a tertiary hospital in June 2017 were enrolled in the control group, while 50 undergraduate nursing students who practiced in the same department in June 2018 were enrolled in the intervention group. The intervention group adopted the flipped classroom teaching mode of the CDIO concept, whereas the control group adopted the traditional teaching mode. After finishing the department practice task, the students in the two groups completed the evaluation of theory, operation skills, independent learning ability, and critical thinking ability. They completed the evaluation of clinical practice ability in eight dimensions, including four processes of nursing procedures, humanistic care ability, and evaluation of clinical teaching quality for two groups of teachers. RESULTS: After teaching, the clinical practice ability, critical thinking ability, autonomous learning ability, theoretical and operational performance, and evaluation of clinical teaching quality in the intervention group were significantly higher than those in the control group (all p < 0.05). CONCLUSION: The CDIO-based teaching mode can stimulate the independent learning ability and critical thinking ability of nursing interns, promote the organic combination of theory and practice, improve their ability to comprehensively use theoretical knowledge to analyze and solve practical problems, and improve teaching effectiveness.

AIMS AND OBJECTIVES: This integrative literature review is to collect what is known about the care of people with dementia when they require a hospital admission for an orthopaedic surgical procedure and to contribute to developing an evidence-base to support nursing practice when caring for people with dementia in an orthopaedic setting. BACKGROUND: People with a dementia diagnosis are increasingly common in acute orthopaedic care settings and the admission exposes people with dementia to risks during their hospital stay. In addition, nurses find people with dementia challenging to care for due to the complexity of dual conditions. Little is known specifically about the care requirements for people with dementia in orthopaedic settings. DESIGN: Integrative literature review. METHODS: An integrative literature review and qualitative deductive content analysis using McCormack and McCance's theoretical nursing framework (Person-Centred Nursing Framework) of nine studies were undertaken. The process of the review was guided by PRISMA checklist. RESULTS: The care environment and resistance either in passive form, or through physical intervention, is common in orthopaedic nursing. Planning and delivering care for physical, cognitive and emotional needs is identified as being difficult, resulting in a lack of inclusion for patients, partly due to communication challenges. Finding ways to implement tailored care plans within standard ward routines proves difficult, and the consequence is a less than optimal care experience with adverse effects on patients characterised by an increase in dementia symptoms. CONCLUSIONS: Care for people with dementia in an orthopaedic setting is complex. It needs to be further studied so that more evidence and supporting literature can contribute to improved care for this group of patients. RELEVANCE TO CLINICAL PRACTICE: This study describes the complexity of providing fundamental care for people with dual conditions of dementia and orthopaedic injury and suggests opportunities for improvement.
